If you offer sedation in your practice, understanding the potential medical emergencies associated are vital. But does your training have to be dental specific? It's a grey area so let's try & shed some light on it. What's the Guidance? The Intercollegiate Advisory Committee for Sedation in Dentistry (it just rolls off the tongue! ) or IACSD provide the Standards for Conscious Sedation in the provision of Dental Care (another snappy title). The standards should give you all you need to know about sedation & the training you need. But does it? IACSD state that the entire sedation team must be trained in ILS as per Resuscitation Council UK guidance. They also state that you don't have to attend a Resus Council course & that dental specific equivalent ILS courses are also acceptable. So you're still left asking... 'WHAT SHOULD I DO? '. You spend A LOT of money to keep your patients safe, but having to throw away equipment without it ever being opened is galling. But why do those items expire? It stands to reason that emergency drugs have expiries as they become less effective & sometimes dangerous over time. But what about equipment? Surely there's no reason for AED pads to go out of date is there... ? Well actually, there's a perfectly good reason why they expire - let's see why... The Lifespan of a Defib Pad The pads that come with your AED are sealed in a packet & likely already attached to your defibrillator ready for use. But they can only be used once. Once opened & used, they need to be disposed of due to (among other things) the adhesive gel that sticks the pads to a patient's chest losing it's stickiness. After approximately 2 years, even without use, the gel begins to naturally break down, lose it's adhesion & potentially begin to peel away from the pad itself. For a defibrillator to effectively analyse your patient, the pads must make a strong connection directly with their skin. If the adhesion of the pads has broken down, the AED won't be able to analyse the patient's heart rhythm effectively. How to Tell the Difference You won't always be able to detect early subtle signs & symptoms of anaphylaxis immediately, but let's look at the key things to consider. 1. The key difference is that anaphylaxis a systemic reaction affecting multiple systems in the body. This often means swelling in other areas away from the site of treatment. Since 2004, I have been involved as a pitch-side sports therapist in rugby, ranging from grassroots to the professional level. This season, I moved into football, working with Pen-y-Bont F. C. who are in the Welsh Premier League.   While I was setting up all the stations during my first game with the football team (a force of habit having worked in rugby), I was told “you will never need that", the reference being to the suturing box, but I set it up ready just in case. A heart attack is a blockage to one or more of the main arteries of the heart. This blockage prevents blood & oxygen from reaching the heart & causes death to the muscle tissue. What causes a heart attack? Most commonly, a heart attack is caused by a build-up of fatty deposits that stick to the arteries over time. Things like smoking, poor diet/lifestyle, diabetes & high blood pressure increase the deposits. This causes a narrowing, which blocks (or initially reduces) the blood from getting to the heart. Shock is the reduction of blood & oxygen to the organs and tissues of the body due to a problem with circulation. If untreated, shock can be fatal. What causes Shock? Shock is usually caused by a drop in blood pressure. This reduces the amount of oxygen to your vital organs and tissues. Entonox is a pain killing/relieving gas made up of 50% Nitrous Oxide & 50% Oxygen. It's a very effective analgesic agent with rapid onset & offset characteristics. Nitrous oxide was discovered in the mid-1700s. Its was first used in America to reduce the pain of tooth extraction & was introduced to Europe in 1867. Studies show 20-30% nitrous oxide has a comparable analgesic effect to 15mg of sub cut morphine. 50% nitrous oxide (Entonox) was equivalent to 100mgs of pethidine. (Chapman et al, The analgesic effects of low concentrations of Nitrous Oxide compared in humans with morphine sulphate). When we use Entonox You should consider using entonox for pain relief after traumatic injuries to your players. You can use it prior to splinting a suspected fracture or painful soft tissue injury in adults & children. Entonox is a self-administered, inhaled analgesic agent indicated for moderate to severe pain. Why we use Entonox in Sport Traumatic sports injuries occur in all sport from grassroot to professional levels. Your club may be well equipped to deal with these injuries, but not all clubs have access to the appropriate equipment or training. This can leave your players at risk and in pain while waiting for the emergency services to arrive. But the knowledge and skills alone aren’t enough. Taking responsibility for your first wound is daunting, but you should be able to manage with confidence even under pressure. Here are 5 quick tips for getting yourself prepared for suturing & wound management: 1. Specialist Training Suturing is a skill you should learn from a medical professional with experience in the field. Any training you attend needs to cover the content required for your role. Do your research and, if necessary, speak with the provider to confirm the content is right for you! 2. Regular Practice You can’t put your skills to the test without regular practice. In 1982, I joined the ambulance service & became one of the first groups of Paramedics to train in the UK in 1987. We weren’t paid any extra for the additional skills and responsibilities, and although it was very stressful, we had the satisfaction of offering a much greater level of care to our patients. At the time there were no automated external defibrillators in the community and for the first time we were able to shock patients at the scene of their collapse. It can happen at any time – not just in work! The structured approach was used by Lynnette Marshall, a dental nurse from Pontcanna Dental Care, Cardiff, to save the life of a lady on a bus. She had attended her annual CPR update training only weeks prior. This example focuses primarily on the importance of checking (or clearing) an airway. Lynnette was on her way home from work on the bus where she noticed some commotion from the seats opposite. Another passenger was becoming distressed whilst trying to rouse their relative who had apparently lost consciousness. Adopting the “D. R. A. B. C. D. E” approach, Lynnette recognised that the situation was dangerous due to the moving bus. She immediately alerted the driver to the situation. He ensured the bus was stopped and all other passengers were asked to get off. Lynnette then tried to see if the lady was responsive, she was not. With the help of another passenger Lynnette was able to get the casualty onto the floor where she could carry out the rest of her assessment. She then assessed the casualty’s airway. She couldn’t see any fluid or blockage. Recognising that the tongue could still be causing a problem, she used the “head tilt – chin lift” technique to move the casualty’s tongue off the back of the airway. But how can we expect head injuries to be taken seriously or fully understood in professional sport, when most people at grassroots don't know the basic considerations? It seems coaches, parents, players, managers (and some first aiders! ) think head injuries are an area that only medical professionals need to worry about. But a small amount of basic knowledge and understanding can help us all deal with head injuries. Electrodes placed on the casualty’s chest deliver a measured electrical shock to the heart to restore natural rhythm. When was the defibrillator invented? The first use of an external defibrillator on a human was in 1947 by Claude Beck. The portable version of the defibrillator was invented in the mid-1960s by Frank Pantridge in Belfast, Northern Ireland. He was a pioneer in emergency medical treatment. How does a defibrillator work? Automatic external defibrillators (AEDs) are small computerized devices that analyze heart rhythms and provide the shock needed for defibrillation. Through electrodes placed on a patient’s chest, a processor inside the AED analyzes the victim’s heart. The machine will not shock unless it is necessary. AEDs are designed to shock only when VF is detected. After the AED analyzes the heart rhythm and determines a shock is required, an electric current is delivered to the heart through the victim’s chest wall through the adhesive electrode pads. The shock delivered by a cardiac defibrillator interrupts the chaotic rhythm and allows it to return to normal. It makes a big difference With CPR alone, the chance of survival after sudden cardiac arrest is less than 5%. When CPR is combined with the use of a cardiac defibrillator within the first few minutes, the chance of survival can increase dramatically to more than 75%. Here are 5 of the main fears you really shouldn't have about using a public defibrillator... 1. Fear of the Unknown... For those that have never been trained to use a defibrillator, or even seen one up close, the thought of grabbing the nearest one and "giving it a go" wouldn't cross their mind. "What if I'm not allowed to use it? ", "What if I hurt someone? ", "What if I hurt myself? ", "I don't know how it works! ". But fear not! Defibrillators are remarkably "intelligent" pieces of equipment that have been designed for ANYONE (that means you! ) to use in an emergency situation. An Automated External Defibrillator (AED) will actually "talk" to you and guide you through how to use it. It really couldn't be easier! 2. Fear of causing the casualty harm... Sudden Cardiac Death (SCD) or Sudden Cardiac Arrest (SCA) is where the heart goes into AN abnormal rhythm, known as an arrhythmia, suddenly and often without warning. The person affected will drop to the floor, will be unresponsive and will not be breathing, this is known as a cardiac arrest. SCD/SCA can affect anyone, but there are certain risk factors that increase the chance of it happening, these are: - Aged between 14-35 Men more than women Putting the heart under physical exertion Afro-Caribbean background - higher incidence of genetic defects In an attempt to identify under laying heart abnormalities and prevent cardiac arrest, the ECB are now providing cardiac screening for all professional cricketers Who is involved and what do they check when performing a cardiac screening session? We combine rescue breaths and chest compressions to try and circulate oxygen around a casualty’s body to the major organs in preparation for defibrillation. What is a cardiac arrest? Cardiac – relating to the heart. Arrest – To stop. Cardiac arrest is when the heart stops pumping blood (and therefor oxygen) around the body. This is often caused by an electrical or mechanical malfunction of the heart, an underlying heart condition, or following a medical emergency. During cardiac arrest, the vital organs are starved of oxygen and the casualty becomes unconscious and stops breathing. Is a heart attack the same as cardiac arrest? No. A heart attack is caused by a blockage in the arteries supplying blood and oxygen to the heart tissue. As the heart is a muscle, it requires oxygen to function correctly. If untreated, this can develop into a cardiac arrest as outlined above. Therefore, a heart attack could be the cause of a cardiac arrest How effective is CPR at restarting the heart? A common misconception is that CPR is designed to “re-start” the casualty’s heart. CPR is effective in circulating blood and oxygen to the vital organs until a defibrillator becomes available to shock the casualty (and theoretically restart the heart). Up to 20% of patients with allergies struggle daily with the fear of a possible asthma attack, anaphylactic shock, or even death from an allergic reaction. In the 20 years to 2012 there was a 615% increase in the rate of hospital admissions for anaphylaxis in the UK. Allergy U. K have reported that an estimated 21 million people in the U. K suffer with some form of allergy. This figure includes 50% of all children. These statistics show that allergies are an increasingly prevalent problem. The severity of the allergy can differ from person to person, ranging from mild and simply inconvenient to a life-threatening anaphylaxis. A person can develop an allergy to any substance and can also become allergic to something they have previously been able to tolerate.   A specific protein contained in the substance will cause the allergic reaction. The competitor returned this year and I caught up with her to find out what happened after the event. I was not expecting the story that followed. Carla, a black belt in Taekwondo, was competing in a sparring contest.  She was wearing the appropriate safety equipment including head guard and gloves. I became concerned about the competitor after she received a blow to her head. She became unsteady on her feet and so the bout was stopped. I removed her from the ring, and she began to complain of nausea and photosensitivity. We moved to a quieter area as the noise levels were beginning to become a problem for her. Following a full assessment, my immediate impression was that she was concussed and should not continue in the tournament. The assessment revealed that it was difficult for her to answer certain questions. She answered Maddocks questions with no problems, it was when I began filling in her details on my treatment record that she struggled.  She was unable to recall her phone number and revealed that she hadn’t been able to remember things for a few days after hitting her head on a concrete archway in a friend’s house. She began to drop her level of consciousness and I became increasingly concerned, especially as this was her second head injury in the space of a week. It was a closely fought day with Cardiff Archers pipping Cardiff Amateur Athletics Club by just 34 points. During the day there were a number of incidents which required medical assistance; from muscular injuries to trips and falls. Luckily none of the incidents required an ambulance or a trip to A&E as appropriate first aid was provided at the venue. This ranged from applying general R. I. C. E principles to player assessment and head injury/concussion advice. It was another interesting and varied event that we were proud to provide support for. First Aid for Sport andExercise (level 1 and 2) or FASE1 & FASE2, are Lubas Medical’s sport-specific first aid courses which teach individuals the skills and knowledge required to deal with injuries in sport. FASE Level one is a six-hour course which is relevant to parents, personal trainers or games teachers etc. who have a role in covering sport. A concussion is an injury to the brain that results in temporary loss of normal brain function. It usually is caused by a blow to the head. In many cases, there are no external signs of head trauma. Many people assume that concussions involve a loss of consciousness, but that is not true. In many cases, a person with a concussion never loses consciousness. The formal medical definition of concussion is: 'a clinical syndrome characterized by immediate and transient alteration in brain function, including alteration of mental status and level of consciousness, resulting from mechanical force or trauma'. (American Association of Neurological Surgeons, AANS). The key words here are 'Brain Injury' as in its simplest form, that is what a concussion is. A concussion could precipitate following a direct impact to the head i. e. blunt force trauma. Or it could occur due to rapid acceleration/deceleration forces. This insult to the brain can cause momentary interruption to cerebral activity. A concussion is not the only medical condition which can result from a head injury, an intracranial haemorrhage is another possible complication. There are many different types of intracranial haemorrhage, all of which can lead to an increase in intracranial pressure, and eventually prove fatal. Initial symptoms associated with a bleed within the cranium may involve lucid intervals, loss of consciousness and headache. All of which are also symptoms which may be present during a concussion. Adrian explains below but what we're looking for is a physio to help the team over the course of the 10 days. Meals and accommodation would be provided and you would be helping a very worthy cause. Over the course of ten days from July the 31stof this year, myself and 7 friends will take on the task of cycling to every NHS children’s’ hospital in England and Wales. We will cover over 1,000 miles whilst visiting 17 children’s’ hospitals in 13 cities including Cardiff, Birmingham, London, Newcastle and Manchester. The inspiration behind this challenge is my three-year-old daughter, Emilia, who has a rare genetic disorder called Alexander Disease which affects the nervous system. It is a form of Leukodystrophy that affects just 500 people worldwide. Unfortunately, the prognosis is not great and children diagnosed with this disease rarely live past the age of 6 or 7. Currently, she works as the Consultant Physiotherapist within the Welsh Rugby Union for Builth Wells and Gwernyfed RFC’s, managing a team of professional and highly skilled Physiotherapists offering pitch side trauma management. I first came across the Lubas Sports Trauma Management course about 9 years ago when I took over the care of a rugby club. Just on the basic confidence front of knowing I was right (and I so do like to be right! ) in managing an injury, and having that backup support if needed (love twitter for that). I’ve been working within club level rugby for nearly 10 years, and I’m thankful that I’ve only had cause for concern about using a spinal board twice. During the most recent incident, the player was ok, but there were key areas that I’d have lacked confidence in had I not attended the Sports Trauma Management course. During the second half of a match, one of our players went in for a tackle. Reviewing the situation the player did not tackle low enough and instead made contact directly through the top of his head. Oxygen has not been shown to have any effect on the sensation of breathlessness in non-hypoxaemic patients (Driscoll et al British Thoracic Society guidelines for emergency oxygen use in adult patients 2008). All seriously traumatised players, Head injury, Spinal injury, Chest injury, Unconsciousness, serious bleeding etc. should be given high flow oxygen as an initial, on-field treatment. Trauma can affect the respiratory system’s ability to adequately provide oxygen and eliminate carbon dioxide in 3 main ways:- Hypoventilation – This can result from a reduced level of consciousness due to head trauma or brain injury, an obstructed airway or decreased expansion of the lungs. (Information from the PHTLS, Prehospital Trauma Life Support, seventh edition 2011) The upper airway should be checked first and any visible foreign objects removed, suction can be used to remove any blood or vomit in the mouth. The head tilt/chin lift should be used to remove the tongue from the back of the airway (or jaw thrust if a spinal injury is suspected). High flow oxygen should be initially applied, for all seriously traumatised players/patients, using a non-rebreather oxygen mask (see image below). Good first aid and/or Sports Trauma Management training combined with the correct use of oxygen can increase the chances of survival following a traumatic injury. All dentists, doctors, practice nurses, dental nurses, hygienists and even receptionists should know how to act properly in a medical emergency. A story that came out this week goes to show the importance of good training. Simon Walker had a heart attack in the waiting room of a dental clinic. It was the receptionist (Charlotte Anderson-Hughs) who was first on scene and with the help of dentist (Chandra Metha) performed CPR and used their defibrillator to restart the man’s heart. The actions of the dental staff saved Simon’s life. Your CPR training needs to include how to properly use AED’s. AED’s in the community: The use of AED’s is becoming more and more common as they are now being installed in more public spaces such as: · Train stations· Airports· Leisure Centres· Supermarkets (ASDA will soon install AED’s in all their major stores) It was recently announced in Scotland that defibrillators will be installed in all NHS dental practices. We’ve provided paramedic support for football, rugby, and cricket games alike, and while you do see different injuries between the different sports, the procedure for treating an injured player is much the same regardless of which pitch you’re on. So what exactly happens when a player sustains an injury? Here’s a step-by-step explanation: The first step, of course, is to assess the injury. Attempting to move the player could be very dangerous if they have experienced a spinal injury, for example, and so we have to know what we're dealing with before proceeding any further. Sometimes, we'll have to administer treatment on the pitch before we take the player to the med room. This could involve anything from sutures to portable breathing equipment - it all depends on the type and severity of the injury. If needs be, we will then take the injured player to the stadium's medical room for further treatment. At this point, we generally hand the reins over to a doctor; this allows us to quickly get back onto the pitch to deal with any further injuries, and it means that the initial injury can be examined by someone who specialises in that particular area. During a US Airways flight from Florida to Arizona, a young baby suffered a seizure and stopped breathing. The report suggests that the infant "turned purple" due to a lack of air; the pilots arranged an emergency landing in Houston, but the child's chances of survival would have been fairly slim were it not for one passenger in particular. As luck would have it, Garrett Goodwin - a professional CPR instructor - was on board when the child stopped breathing. In the Sky News article, Mr Goodwin explains the techniques he administered to save the young boy's life: chest compressions, clearing the airways, rescue breathing and stimulation. It's a surprisingly simple process, but it can (and frequently does) make a huge difference. Happily, the procedure worked, and we can only assume that the whole cabin breathed a huge sigh of relief when the baby woke up crying. Without that one man and his resuscitation training, the story could easily have ended in tragedy. It just goes to prove what we've said all along: CPR, a technique that takes mere hours, to learn, can save lives in a very real sense. The Lubas paramedics have seen a lot of injuries out on the playing field, and these range from minor bumps to serious wounds and even worse. Here are some of the most common injuries that we come across: Concussion - If a player sustains a blow to the head, it may cause concussion, leaving them temporarily dazed and confused. Cuts and Wounds - Obviously, this is quite a broad category. It could be a small cut, in which case the player will usually get straight back onto the pitch, or it could be a more major wound, with the injured player bleeding heavily. Broken Bones - People who play contact sports like rugby can easily sustain a fracture. We use a variety of splinting methods to deal with these injuries. Sprains - This one is very common in football matches. A sprain occurs when ligaments are stretched further than they're supposed to go; depending on the severity, sprains can put players out of action for some time. This is just a sample of what we deal with. To give you an idea of what we do on a match day, we thought we'd talk about our role in the last big Welsh rugby match: the 6 Nations opener against Italy on Saturday the 1st of February. Whenever there’s a big game at the stadium (and as far as Cardiff is concerned, they don’t come much bigger than international rugby), we have to arrive a solid 2 hours before kickoff – partially because we need time to set up, but also because the roads would be closed if we left it any later! Once we’d zig-zagged through the crowds and made it to the stadium, our first job on Saturday afternoon was to set up the players’ medical room. This is where the lads end up if their injuries require more medical attention than we can properly provide out there on the turf. For example, if a player looks like he might have broken something, we’ll stretcher him off to the medical room, where his injury can be properly assessed. The Millennium Stadium’s medical room contains four beds, and it’s kitted out with all the monitoring equipment that you’d need to keep track of a player’s blood pressure, heart rate, and oxygen level. After our home victory against the Italians last week, our fair nation was a beacon of optimism, positively brimming with talk of a record-breaking, coveted third 6 nations trophy in a row. This was all until the Welsh squad came out to face 15 green jersey-clad men with only one thing on their mind – to crush Welsh hopes of a grandslam! It is fair to say that the Welsh performance was flat; lapses in discipline, dropped passes, missed tackles, and turnovers all handed the Irish the reins, which they were more than ready to steer to victory. A near faultless display by Jonathan Sexton, the Ireland fly-half, and some unfortunate Welsh errors saw the first half finish 13 – 0 to Ireland, so a comeback was never going to be easy. After Gethin Jenkins was penalised Sexton then made another conversion to take it to 16 – 0 – things were looking increasingly impossible for the defending 6 Nations champions. Though our main man Leigh Halfpenny was able to get us on the scoreboard, Paddy Jackson’s final conversion was the finishing blow in what turned out to be a bit of a mauling at the Aviva Stadium. The Millennium Stadium hosted the first game of the 2014 Six Nations Championship on Saturday, and as we may have already mentioned, our guys had an important role to play. The Wales-Italy game was a mercifully quiet one as far as injuries were concerned, but the Lubas lot still got right into the spirit of the occasion, joining in with the anthems and generally soaking up the famous Millennium Stadium atmosphere. We were hoping that the scoreline would be slightly more one-sided, but hey, a win is a win! Still, rugby wasn’t the only sport we were involved with on Saturday. We also provided medical cover for Cardiff City’s match against Norwich, which meant that we witnessed a little bit of history. Craig Bellamy, who scored an important equaliser for Cardiff in the second half, is now the first player to score for seven different Premier League football clubs*. Congrats, Craig! More importantly, of course, Cardiff secured a good three points (the game ended 2-1 to the home team) and yanked themselves up from the bottom of the table. Still, injuries do happen – our paramedics are all too aware of that – and if you don’t take certain precautions before a rugby match, they’re all the more likely to occur. Here are four ways for you to minimise the chance of a casualty: Warm Up ProperlyIt’s important to engage in a thorough warm-up before any form of exercise, but it’s particularly crucial before playing rugby. If you’re preparing for a full match, you’ll probably want to spend a solid thirty minutes on the warm-up; be sure to do lots of stretching and pay particular attention to your back muscles, as these will be under a lot of stress during the match.
